There is no Multiplayer in the Watch Dogs as a separate game the same way as in AC4. All online interaction are within your main game. Game developers don't call it "single player" or "multiplayer", they call it "seamless online".
Can anyone come and hack your game at any time?
No. Gate is closed when you are in Story mode or in a Mission. Intrusion is possible only when you are in free roaming, but also you have a shield that protects you from hackers. This shield is adaptive and if you participate more in online activities (multiplayer) - you will become more open for hackers.
Is cross-platform online communication possible?
No. It is not possible from PS4 hack the game on Wii U. But from the ctOS Companion App (which is free) you can join the game on any platform which is in free roaming mode.
Whom you can hack?
For Intrusion and Tailing system pairs you with the pool of players based on your skill level. You cannot select anyone outside of this pool.
For the Watch Dogs Racing multiplayer and for Watch Dogs Decryption multiplayer - you can select public matches or make a private match and invite your friends only.
Skill unlocked during multiplayer - can be used in Story mode and vice-a-versa.
You will be Aiden Pearce and will wear your "story" outfit for all multiplayer sessions. You cannot select another persona for yourself in multiplayer, but you can select faction skin, tags, etc.
In Watch Dogs Racing Multiplayer players can select different cars and race on different tracks, but all within your "story mode" world.
By doing well in Multiplayer mode you will rise your Notoriety level. If you have been hacked - you will loose some of the Notoriety points. Higher Notoriety levels will unlock new skills. Once you achieve certain level of Notoriety - it transfers into the Watch Dogs Leaderboard. Confusing, huh?
